In order for OEG staff to provide you with effective support in case of illness or injury, we need you to provide us with any relevant medical information. All students will need to print a copy of the Medical form. If you have asthma, please also print a copy of the Asthma form. If you have any allergies, please print a copy of the Allergy form. The relevant forms must be completed in legible handwriting and signed by your parent or guardian, then returned to school as soon as possible.
